Any licensed physician may prescribe cannabis flowers and extracts, regardless of their medical specialty (except dentists and veterinarians), as long as there is a medical necessity. It must be ensured that other treatment options have been exhausted and proven ineffective, and that the prescription complies with current legal regulations. To receive a prescription for medical cannabis in Germany, patients must first schedule an appointment with a doctor’s office or use a telemedicine service. The doctor will assess whether a medical indication is present and whether other treatment methods have already failed. If cannabis is deemed appropriate, a prescription will be issued—either as a public health insurance prescription (with prior approval from the health insurance provider) or as a private prescription, which the patient pays for themselves. The prescription can be filled at a local specialized pharmacy or through an online pharmacy. After the initial prescription, follow-up prescriptions can be obtained from the same or a different practice.

If the costs of cannabis therapy are to be covered by public health insurance, prior approval from the insurance provider is usually required before issuing the prescription. This requirement does not apply if a private prescription is issued—in this case, the patient covers the costs themselves. Since April 1, 2024, medical cannabis is no longer prescribed on a narcotic prescription form but via a standard electronic prescription (e-prescription). Certain groups of physicians with specific qualifications may issue prescriptions covered by public insurance without prior approval. In specialized outpatient palliative care, no approval is required.

Important: Before issuing a prescription covered by public insurance, doctors should ensure that costs will be covered by the patient’s health insurer to avoid potential repayment claims. A private prescription can be issued without consulting the health insurer, and the patient will bear the treatment costs themselves.

Health insurance may cover the cost of treatment with cannabis medications if:

  • A generally accepted, standard medical treatment is not available,
    or
  • In individual cases, based on the justified assessment of the treating physician, alternative treatments are deemed unsuitable due to the expected side effects and the patient’s medical condition.
  • Alternatively, there must be a not entirely remote prospect of a noticeable positive impact on the course of the disease or on serious symptoms.
